Fall Sports
Please remember you must have your sports physical and sports registration form completed and submitted to Magnus Health in PRIOR to the first day of tryouts/practice.
Step #1 - complete and submit the Pre-participation Physical Form. Page 1 is filled out by the student and a parent. Please ensure the page is filled out in its entirety, including the student and parent signatures at the bottom of the page. If either of the signatures are missing, your form will not be approved. Please print out page 2 of the form and take it your physician to have him/her complete during the physical examination. Once the form is complete, please upload BOTH PAGES to Magnus Health (access through PowerSchool).

Step #2 - complete the Athletic Registration Questions on Magnus Health (access through PowerSchool).
Step #3 - pay the Athletic Fee online. This can be done once your daughter has officially made the team. A uniform will not be distributed until the athletic fee is paid.
